Immunization Schedule

A current immunization schedule is listed below. Please be advised that immunizations are dynamic and change is to be expected. A parent or guardian must be present to provide consent for vaccines to be administered to any patient less than 18 years of age.

*After the age of 6 months, an annual Influenza is recommended

Please Note:

  • Schedule your child’s checkup now and avoid the rush.

  • Kindergarten immunizations can be given any time after your child turns 4 years old.

  • Please ask our staff at your child’s next checkup for a copy of the FL Immunization form.

  • If you call or come by the office to request a copy of your child’s FL Immunization form, please allow us 72 hours to complete the request.
