Patient Responsibility

We expect you to provide us with your child’s and your family’s medical history, as well as information about any care they obtained outside of our Practice to include current medications (prescribed and OTC), recent test results, visits to other doctors and health care providers, behavioral health services, hospitalizations, and emergency department visits. We also expect you to update your demographic information upon any changes. We encourage you to actively participate in your child’s care by bringing a list of questions to each Well Child appointment, learning about their conditions and following any established care plan that you and the medical team agree upon. If you do not understand something your medical team says, ask for them to explain it in a different way. Talk openly with your care team about your experience so that we can keep making your child’s care better.

Patient Centered Medical Home

WELL CHILD CHECK UPS

Preventive care is a crucial part of your child’s total health care. Well-child check-ups help to evaluate & monitor your child’s growth and development. These visits are important, even when your child is healthy. They are crucial in making sure your child is growing and developing properly, and to provide any needed services before the time frame for mitigation has passed.

IMMUNIZATIONS

Immunizations are discussed and given (if appropriate) at the Well Child visit. We follow the American Academy of Pediatrics Guidelines for the schedule of these well visits and immunizations.

For Parents

Dear Parents,

As parents, we all desire the best for our children – the best family situation, the best education and the best medical care.

The concept of the “medical home” has always been at the heart of the practice of medicine. Continuity of care is undeniably critical to providing high-quality health care, providing more in-depth understanding and knowledge of your child’s health needs beyond what is obtained in a single limited visit (at an outside walk-in clinic).

Dr. Jaafar and the staff at Safari Pediatrics pledge to provide the best possible care for your children throughout their young lives. We are available 24 hours a day by phone and our offices are open five days a week. We offer same day appointments for sick and well children (if time permits) and take care of simple and complex problems.

We do not try to be everything to everyone, but we do try to be the best at what we do. We hope to enjoy a long-standing relationship with your family and we appreciate your loyalty.

Patient Portal

We want to make your life a little easier however we can, from giving your children the highest quality medical attention while they’re in our office, to providing you with advice and answers that will help you care for your child’s day-to-day medical needs. With today’s advanced technology, we are constantly finding more efficient ways to serve your family, and our Patient Portal is no exception.

Newborns and Expectant Moms

Choosing the right pediatrician for your new baby is a very important and personal decision.

If you are expecting a baby, we invite you to schedule a free “Meet the Doctor” appointment. This optional first visit allows expectant parents to meet with our doctor, ask any questions they may have, and take a tour of our office.

We have taken special care to develop the services new parents need in caring for their newborn.

Safari Pediatrics is dedicated to providing excellent clinical care and outstanding customer service. We strive to foster a caring partnership with families. Our goal is to provide a medical home to our patients, where families can expect compassionate and comprehensive pediatric care, convenient access to quality after-hours advice, and professional coordination of care between community specialists and services. We focus on the health and well-being of the whole child and believe in the importance of continuity of care (including regular well care and immunizations). Our comprehensive services are provided to address a wide range of patients’ needs, including preventative medicine, acute and chronic illnesses, disabilities, developmental and behavioral issues, multiple births and prematurity.

Your First Visit

Please contact our office during business hours to schedule your newborn’s first visit. We prefer to see your baby 2-4 days after being discharged. During this visit we will evaluate your newborn’s feeding, weight, or any possible issues.

For your visit, please bring your insurance card, child’s shot record, any discharge paperwork, and arrive 30 minutes early to complete the necessary patient information forms. To ensure insurance coverage for your new baby, it is important to add your newborn to your insurance plan within 30 days of birth and choose our physician as your baby’s primary care physician (PCP) if a PCP is required. Check with your insurance company for details on how to add your child to your policy.
